Username Tips: The username and password provide access to editing or making new records. Contact HCCREMS if you do not have or have forgotten your user name and password. Generally each organisation has a single login account.
Organisation Tips: Your organisation's name.
Work phone Tips: Area code and then number. Used for database admin only. Not made public on the database.
Mobile phone Tips: Not mandatory. Used for database admin only. Not made public on the database.
E-mail Tips: Email address. Used for database admin only. Not made public on the database.
